Media Consent Form

About this form

Learn and Thrive and 21 Together sometimes take photos and videos during our sessions and events. We would love to be able to use these to share our work, raise awareness and raise funds so we can keep making a difference. 

This form explains how we might use images and video and asks for your permission. 

You do not have to say yes. Saying no will not affect your or your child's involvement in any of our activities. 

How we might use images and video

Photos and videos may be used for: 

• Our social media channels  

• Printed materials such as leaflets, reports and newsletters  

• Fundraising and sponsorship  

• Impact reporting

• Events, presentations and exhibitions  

• Our websites 

Both Learn and Thrive and 21 Together may use the images for the purposes above.

Notes on how we use and store these images

• We will never use images in a way that is disrespectful or that puts anyone in a negative light.

• We will only use a first name alongside an image, and only where you have given your consent to all use of photos and videos.

• Learn and Thrive and 21 Together each hold a copy of any images and videos, and each are responsible for their own use; link to our privacy policy (https://www.learnandthrive.org.uk/privacy); the form is kept for as long as any image is in use plus six years; unpublished images are deleted at the end of the project unless consent is "yes to everything"; rights to access, correct, delete, object, withdraw, and complain to the ICO.

• If you would like to withdraw your consent, you only need to tell either Learn and Thrive or 21 Together and this will be communicated to the other internally. Both charities will cease use immediately and remove online copies within 30 days. We cannot recall printed items or copies already sent.

Completing this form

How you complete this form will depend on the age of your young person:

• Under 16: the parent or guardian gives consent. We also ask the young person, and a no from them is a no.

• 16 and 17: the young person gives consent. The parent or guardian signs to say they support the decision.

• 18 and over: the young person gives consent for themselves. If they can't make this decision even with support, the parent, guardian or carer completes Section 1 to tell us what you believe the young person would want, and we will only use images where the young person is happy to be filmed and shows no reluctance or distress.
